Objective:

To investigate the reproducibility of the Frankfort horizontal plane (FHP) established using CBCT-based three-dimensional reconstruction and to evaluate reproducibility errors generated by CBCT and lateral cephalometric reference-construction workflows in patients with facial deformities.

Approach:
  • Study Design: 27 patients with facial deformities underwent both CBCT scanning and lateral cephalometric radiography. 3D craniofacial models were reconstructed, and FHP was established.
  • Data Analysis: Landmark positional errors and plane angular errors were calculated. Intra-class correlation coefficients (ICCs) assessed reproducibility, Bland–Altman analysis evaluated agreement, and paired t tests compared reproducibility errors.

  • CBCT 3D reconstruction method showed good reproducibility for all landmarks and planes in establishing FHP.
  • Lateral cephalometric measurements had larger reproducibility errors compared to the CBCT workflow (p < 0.05).
